1. Resolution and Proxy Information & Form
A special general meeting is being called by the Directors of GPNNT for members to consider the windup of GPNNT and to amend the Model Constitution of the NT Medicare Local to enable members as a collective to participate in the NT Medicare Local.

Time:   12:15pm registration of voters and Teleconference Participants

Meeting including briefing session: 12:30pm - 1pm

Members have the right to express their vote by way of proxy. Members who wish to appoint a proxy, must ensure the proxy is a current member of the company and may by default appoint the Chair of the meeting. Appointment of a proxy must be in writing.

If you would like to appoint a proxy to act on your behalf then go to this link Instrument Appointing a Proxy and download the document and complete the proxy form and have your proxy sign the form as well as yourself. 

All signed proxy forms must be returned by scanned email or by fax by the C.O.B Wednesday 16th May 2012. Details on how to return are on the proxy form.

Special Resolution to be considered by members
The Board of Directors of General Practice Network NT Ltd (GPNNT) recommend to members of the company, that they:

  1. wind up GPNNT at a time convenient to GPNNT and
  2. support the GPNNT Board in its endorsement of a proposed Constitution of the NT Medicare Local that will;
  • enable GPNNT’s existing membership (and anyone who in the future complies with its requirements for membership) to apply for membership in the NT MEDICARE Local as a special category of member (GPNNT Category of Member),
  • ensure that existing membership (and future persons complying with GPNNT current requirements for membership) are granted membership to the GPNNT Category of Members ‘as of right’,
  • enable GPNNT Category of Members to exercise as a collective the ‘vote’ that each of the other Foundation Members has in the NT Medicare Local.
